CSB Reveals ‘Critical Deficiencies’ Behind Near-Fatal Ammonia Leak at Virginia Food Processing Plant

The U.S. Chemical Safety Board (CSB) recently released its final report on a dangerous ammonia leak at Cuisine Solutions, a food processing plant in Sterling, Virginia. In July 2024, about 125 kg of ammonia escaped from the facility, leading to serious health issues for employees.

The CSB found major flaws in the plant’s safety systems. Notably, the facility lacked alarms or automatic shutdowns, which are essential for handling ammonia—a common refrigerant in the food industry. This oversight proved costly; ten workers lost consciousness, and 33 needed hospitalization, with one in intensive care.

Cuisine Solutions specializes in packaged meals, using a cooking method called sous vide. Here, ammonia helps cool the water used in the cooking and freezing processes. However, the CSB’s investigation pointed to a malfunction in a heat exchanger that caused a dangerous build-up of pressure, resulting in the ammonia release. Unfortunately, there were no records to explain what went wrong.

One troubling detail was the makeup of the ammonia vapour. It contained liquid aerosol, making the gas denser and causing it to hover near the ground. This meant that employees could not escape safely, as the evacuation routes went through the toxic cloud. The CSB reported that the ammonia concentrations posed immediate health threats, yet the company’s emergency action plan didn’t prepare workers for such a scenario.

The CSB made several recommendations, urging Cuisine Solutions to update its emergency plans, implement alarms, and train staff specifically for ammonia leaks. Steve Owens, CSB chair, emphasized the need for proper safety procedures to prevent tragedies triggered by toxic releases.

While the CSB has been influential in enhancing industrial safety since its formation in 1998, it faces funding challenges. In July, the Trump administration decided to remove its budget, marking the end of its regulatory powers. The agency has successfully investigated over 170 chemical incidents and issued more than 1,000 safety recommendations.

This incident highlights a broader issue in industrial safety, especially regarding hazardous materials. According to recent studies, facilities handling toxic substances often overlook the need for comprehensive emergency response plans. It’s essential for companies to prioritize safety measures, ensuring both protocols and equipment are in place, especially when dealing with risks like ammonia.
