In Glendale, California, an alarming case has emerged involving Dr. Violetta Mailyan. Over the last few years, she collected an astounding $25 million in Medicare reimbursements for Botox injections. This amount is more than any other provider in the country. Unfortunately, it turns out this was part of a large fraud scheme.
Federal prosecutors revealed that Mailyan submitted fraudulent claims for Botox treatments that were either never performed or not medically necessary. According to the U.S. Department of Justice, she abused Medicare’s rules surrounding Botox, which is only covered for specific conditions like chronic migraines or muscle spasms.
From 2019 to 2025, Mailyan operated Healthy Way Medical Center and faced accusations of billing for procedures while on vacation, even collecting claims when her clinic was closed. In one shocking instance, she billed for services tied to a patient who was incarcerated.
This case is considered one of the largest Botox fraud schemes in U.S. history. Patrick Grandy, the FBI’s Los Angeles Field Office director, stated that Mailyan falsified patient diagnoses and attempted to mislead investigators with fake records. Assistant Attorney General Colin M. McDonald highlighted the importance of a data-driven approach in identifying such frauds, ensuring accountability among healthcare providers.
Interestingly, Mailyan reportedly used her ill-gotten gains to fund luxurious vacations and buy extravagant items, like a rare 17th-century crossbow. Her assets, including a Tesla Cybertruck and properties worth over $7 million, have been seized as part of the investigation.
Mailyan has been convicted of nine counts of wire fraud and three counts of obstruction of justice, facing nearly 200 years in prison when sentenced in September.
This case underscores serious issues within the healthcare system, particularly regarding fraud in Medicare. According to a report from the U.S. Department of Health and Human Services, healthcare fraud costs taxpayers billions each year.
