Celebrating Innovation: BD, Direct Relief, and NACHC Honor Four U.S. Community Health Centers Advancing Care Excellence

Four U.S. community health centers are set to make strides in patient care thanks to a $225,000 award from BD Helping Build Healthy Communities. This grant will help these centers roll out pharmacist-led medication management programs, focusing on patients at risk for chronic diseases.

Here’s what each center plans to do:

  • Community Health and Social Services Center (Detroit, MI): They aim to help patients with Type 2 diabetes by screening for conditions like peripheral artery disease. Bilingual healthcare workers will help bridge communication gaps.

  • Jane Pauley Community Health Center (Indianapolis, IN): They will create a care model that combines clinical pharmacists and community health workers. This will address both health issues and everyday challenges like nutrition and transportation.

  • Family Health Matters Community Health Center (Anaheim, CA): They plan to enhance their services for Spanish-speaking patients and focus on health literacy, access to food, and community support through group visits.

  • Southwest Virginia Community Health Systems (Meadowview, VA): This center will expand its services to include management for chronic kidney disease, improving patient screening and care.

Since 2013, BD has partnered with Direct Relief and the National Association of Community Health Centers to support low-income and uninsured communities. They’ve invested $22.6 million to help 56 health centers across 20 states.

One expert in community health says these initiatives not only improve patient outcomes but also empower individuals to take charge of their health. By integrating social support with medical care, these centers are addressing the root issues that affect health.

Moreover, a recent survey found that about 80% of patients benefit from integrated care models that tap into social determinants of health. This suggests that addressing these factors is crucial in the fight against chronic diseases.
