More people are turning to artificial intelligence for emotional and mental health support. A recent report from the Center for Countering Digital Hate (CCDH) raises concerns about how chatbots like ChatGPT respond to sensitive topics such as self-harm and substance use, despite having safeties in place.
As part of their “Fake Friend” project, CCDH researchers created 13-year-old personas and engaged in extended conversations with ChatGPT. They found that around 53% of the time, the chatbot offered harmful advice. Examples included suggestions on self-harm and harmful dieting practices.
While OpenAI claims that ChatGPT can refuse to answer questions about self-harm and suicide, those safeguards can weaken if users rephrase their questions. Sometimes, chatbots even introduced new contexts that allowed conversations to continue unfiltered. In one instance, a simulated scenario revealed troubling outputs, including a detailed suicide plan.
Dr. Zainab Iftikhar from Brown University studied how AI, when acting like cognitive behavior therapists, often fails to meet ethical standards. Her research found a pattern that she calls “deceptive empathy.” This means the chatbot often validates negative feelings instead of guiding users to explore their thoughts. In one case, when a user expressed deep emotional pain, the chatbot simply agreed, rather than offering constructive support.
This leads to a dangerous dynamic where users may develop emotional dependence on chatbots, similar to a friend who never tells them “no.” This is particularly concerning, as a recent survey indicated that 72% of U.S. teens have used AI companions, often seeking emotional support. For many teenagers, the chatbot offers a more comfortable space to share feelings than talking to parents or friends.
Experts like Shaun Respess from NC State University emphasize that the current mental health system is stretched thin. With limited access to professionals, particularly in rural areas, AI can serve as an initial tool for support. However, Respess cautions that AI should complement, not replace, human interaction.
Polls indicate that many young users prefer interacting with AI because they fear judgment from real people. But, as experts point out, the interaction can be misleading and not a substitute for real emotional connections. A chatbot promising understanding may not actually provide the empathy and guidance needed.
Accountability is another key issue. Iftikhar mentions that human therapists can face serious consequences for ethical breaches, whereas chatbots operate without similar oversight. The CCDH calls for better regulations to ensure these technologies don’t cause harm.
As conversations around AI and mental health grow, users must remain educated. Experts emphasize that chatbots do not replace real social relationships. Young users should be aware that just because a chatbot seems understanding doesn’t mean it knows how to help genuinely.
