Jersey’s Family Nursing & Home Care has teamed up with Graphnet Health to launch a groundbreaking remote monitoring service. This initiative marks a significant step in digital health care on the island, thanks in part to funding from the Impact Jersey CareTech Challenge, managed by Digital Jersey.
With this new service, Family Nursing & Home Care (FNHC) will use Graphnet’s Remote Monitoring solution, backed by Luscii, to keep track of the health of frail and vulnerable residents at home and in community settings. The project is part of a larger effort financed by an £800,000 CareTech grant aimed at improving care for older residents in Jersey.
Rosemarie Finley, CEO of FNHC, shared insights on how this initiative will benefit the community. She said, “We witness every day how frailty, falls, and chronic conditions affect our patients. This project will help us monitor people in their homes more closely, allowing us to detect early warning signs and intervene sooner, all while easing the burden on both patients and staff.”
Amy Taylor, COO at Digital Jersey, emphasized the project’s potential: “The CareTech Challenge aims to support practical digital solutions for older residents. By collaborating with FNHC and Graphnet, we aim to integrate effective remote monitoring technology into Jersey’s healthcare system. If successful, this model could expand to benefit even more residents in the future.”
Tom Muller, from Graphnet Health, expressed enthusiasm about the project. “This deployment is a unique opportunity for Jersey to leverage data and digital tools in patient care. Our solution will allow FNHC staff to focus their attention where it’s needed most, assuring patients that their health is under constant watch.”
