SAN ANTONIO – In January, some San Antonio kitchens had some unwelcome visitors. The San Antonio Metropolitan Health District reported troubling findings from health inspections at local restaurants.

In our ongoing series, Behind the Kitchen Door, we take a closer look at restaurant hygiene and food safety. Here’s what we found this month:
Karolina’s Hacienda
On January 8, inspectors rated Karolina’s Hacienda, a Mexican restaurant, at 83. However, they noted several issues that might leave diners concerned. Located at 11510 U.S. Hwy. 281 South, the restaurant is under new management. Still, the inspection found cold food items stored at unsafe temperatures.
This resulted in the disposal of many pounds of food. Unfortunately, inspectors also discovered food contaminated with pests. When asked about these problems, an employee claimed they had been resolved but did not provide further comment, saying they were following guidance from the health inspector.
Dr. Anita Kurian, Deputy Director of Metro Health, emphasized that businesses should feel free to discuss their inspections with the media, stating that any reluctance from restaurant staff does not reflect Metro Health’s stance.
Little Woodrow’s Braun
At Little Woodrow’s Braun, a bar on 9840 West Loop 1604 North, inspectors returned for a follow-up after scoring an 84 on January 10. The inspection revealed ten violations, including sightings of live roaches in the kitchen. Additionally, staff members were reminded to wear proper hair coverings.
Bertha’s Mexican Restaurant
On January 6, Bertha’s Mexican Restaurant at 2629 West Martin Street received a score of 85. The inspector found pest droppings in the storage area and noted that some food items were uncovered in the cooler. There were also concerns about staff handling ready-to-eat foods without gloves.

Score Guide
Understanding the scores:
- 100-90 = A (Very Good to Acceptable)
- 89-80 = B (Acceptable to Marginal)
- 79 or lower = C (Marginal to Poor)
