The recent Ebola outbreak, declared a public health emergency by the World Health Organization, has raised serious concerns. As of now, over 600 suspected cases and 139 deaths have been reported. Most of these cases are in northeastern Democratic Republic of Congo, a region grappling with years of conflict. Uganda has also reported two cases in its capital.
Dr. Abraar Karan, an infectious disease expert from Stanford University, described the situation as a “perfect storm” for the spread of the virus. The strain identified is rare, and there are fears it may have been circulating undetected for months.
How Do Outbreaks Start?
Ebola outbreaks usually begin in parts of East and West Africa, primarily in rural areas. The Democratic Republic of the Congo has reported the most outbreaks since 1976, with this being the 17th. But the source of the virus remains unclear. Dr. Karan suggests that humans might contract Ebola from eating bats or being exposed to bat droppings, especially in caves. Other animals, like certain deer and primates, have also tested positive for antibodies related to the virus.
Once a person contracts the virus, they can spread it to others. This initial “spillover” is critical in the transmission cycle.
Understanding Symptoms
Dr. Nahid Bhadelia treated over 500 Ebola patients during the West Africa outbreak. She observed that symptoms can vary widely. Initially, Ebola can present like the flu, with mild symptoms such as nausea and fever. This can make early detection tricky. However, as the illness progresses, it can lead to severe outcomes like shock and organ failure.
Contrary to Hollywood portrayals, the most common symptoms include severe vomiting and diarrhea, often bloody. Bhadelia noted a significant difference in mortality rates depending on the quality of medical care received. For instance, during the West Africa outbreak, mortality was around 50-70%, while in the U.S., it dropped below 20% for those treated adequately.
What About the Current Strain?
The current outbreak features the Bundibugyo strain, which historically has a lower fatality rate than the Zaire strain responsible for the 2014-2016 outbreak. However, there are few past cases to draw data from. Unfortunately, no vaccines or treatments specifically target the Bundibugyo strain, which worries many experts.
Dr. Daniel Bausch pointed out that effective control can still be achieved using established public health methods like rehydration and contact tracing. Yet, the absence of medical options remains a significant challenge.
Contagiousness of Ebola
Fortunately, Ebola is not airborne like COVID-19. One infected person typically spreads the virus to about two others. Direct contact with bodily fluids—like saliva, blood, or vomit—is how the virus mainly spreads. Patients are not contagious until they show symptoms, which highlights the importance of proper infection control.
Tragically, during previous outbreaks, funerals greatly contributed to the spread, as cultural practices often involved close physical contact with the deceased.
Reasons Past Outbreaks Fade
Many Ebola outbreaks fizzle out for two main reasons: they usually occur in rural areas, limiting their spread, and the high fatality rate often means many patients die before transmission can occur. However, experts are increasingly concerned. The 2014–2016 outbreak escalated as the virus spread to urban centers.
Current Concerns
Experts are worried about the potential for this outbreak to escalate due to the lack of vaccines, its delayed detection, and its spread into large cities. There’s a fear that the outbreak is much larger than currently diagnosed, considering how many healthcare workers have been infected.
Regional health challenges complicate response efforts. The outbreak emerged in a conflict-ridden area with limited healthcare resources, making contact tracing difficult.
Public Safety
So, what does this mean for people outside the region? While there is concern that cases could emerge elsewhere, experts like Dr. Bausch believe that the risk of an outbreak in high-income countries like the U.S. is low. The virus’s transmission methods keep the general public safe, especially since effective healthcare systems are in place. The focus remains on monitoring and caring for those at higher risk, such as healthcare workers.
This situation serves as a reminder of the importance of preparedness for infectious diseases. While the risks can feel distant, staying informed and maintaining broader health practices—like getting a flu shot—remain essential. For now, the situation calls for vigilance and adequate public health responses rather than panic.
